During one's lifetime

Pledger's Form

  • Review the Pledger's Form and No Objection Certificate.
  • Print or save the form after reviewing it.

Fill the form

Print the form and fill it in pen. Attach a passport-size photo of the pledger.

Submit the pledge

  • Scan the filled form along with a copy of the pledger Aadhaar card.
  • Email the scanned documents to brainbank.neuropath@nimhans.net.
  • Send the filled forms along with a copy of your Aadhaar card to: 004, Human Brain Tissue Repository (HBTR), Neurobiology Research Centre (NBRC), NIMHANS, Bengaluru 560029

Receive confirmation

Upon verification, you will receive:

  • A pledger number.
  • An acknowledgment letter and brochure confirming your pledge.

At the time of donation

Keep documents ready

  • Death certificate
  • Photo and ID proof of the deceased
  • Brain donation card (if registered)
  • Copy of the Medical Records (if any)

Reach the NIMHANS mortuary

  • Postmortem usually takes 1-2 hours after arrival.
  • Utmost care will be taken to ensure there is no disfigurement, and your loved one will be returned with utmost dignity and respect.

Collect documents after postmortem

  • Acknowledgment for brain donation
  • Death certificate (if admitted at NIMHANS)
  • Copy of Informed consent
